Information for Mental Health Provider Agencies

Wright School is a state-operated program fully funded by the Division of Mental Health. All Wright School referrals must be approved by the local management entity (LME). It is the LME's responsibility to determine whether or not less restrictive local resources are more appropriate before making the referral to Wright School. Value Options is not involved in authorizing Wright School's treatment services.

Follow the steps below to ensure a complete referral packet:

1.	Click here to download the Wright School Referral Form and the Information Checklist and complete them. You will need to provide the supporting documents requested. (Note: These forms are in PDF format. To read PDF files, you need Adobe Acrobat Reader. If you do not have this free program, please click here to download.)
2.	Schedule a meeting with the LME's Care Review/Community Collaborative. If they approve the referral to Wright School, have the designated persons authorize approval at the bottom of the Information Checklist.
3.	Upon completion of the previous two steps, mail or fax the referral packet to:
